Organisation of territory assistance to diabetics: training of nurses, a real life experience

Introduction: the increasing prevalence of DM and the complexity of its management is a challenge for diabetologists. The organization of local assistance to diabetic patients needs the government by diabetologist with a structured project in which dedicated nurses are essentials in a multiprofessional team. Objective: to develop a model of health local assistance to DM involving multiprofessional team with nurses on the territory dedicated to diabetes care, firstly we organize a structured course focused on multidimensional approach to DM from diagnosis to therapies and screening of complications expecially diabetic foot (DF). Secondary we evaluate nurses’ satisfaction. Subjects and Methods: theorical-practical course involving nurses operating in the territory, in “case di comunità” in ATS Brescia with frontal lessons and role playing (10 hrs), practical exercitations in DF and general diabetic clinic (5 hrs, respectively). Six months later, a survey with google form about satisfaction, utility and unmet needs was performed. Results: 29 nurses participated in the course; 20 nurses answering the survey, all of them were female, 70% aged 40-60 yrs; 55% had long standing professional experience (>20 yrs; 95% (n=19) in hospital setting, but only 35% (n=7) in local health care) and most of them operated in “case di comunità” for less than 5 yrs. The most appreciated sections of the course were: practical exercitations, management of new diagnosis (diet and bureaucracy), glucose sensors and DF (inspection for ulcer risk classification and triage of severity foot infection). Surprisingly there was less interest on insulin therapy and Self Monitoring Blood Glucose (SMBG). Conclusion: a structured nurses’ formation project improves the approach to new DM diagnosis and first level care of DF. However management of insulin therapy and education of SMBG must be optimized in “case di comunità” for creating a better hospital-local assistance for chronic patients.
