Aim: This study aims at evaluating the metrics of glycemic control in people with type 1 diabetes aged 55 years or older, on treatment with an artificial pancreas. Methods: This is a retrospective observational study of individuals with type 1 diabetes using advanced hybrid closed loop (AHCL) systems for at least six months and followed at a University Hospital of Southern Italy. Data on metrics of glucose control were collected at different times: baseline (Time 0), after three (Time 1), and six months of treatment with artificial pancreas (Time 2). The primary endpoint was the change in glucose management indicator (GMI) across the different time points. Results: A total of 42 subjects were included in the study. A significant increase of TIR (time 0 vs time 3, 47 (37, 62)% vs 67 (59, 75)%, p<0.001) associated with a significant reduction of GMI (time 0 vs time 3, 8 (7.3, 8.7) vs 7.1 (7.0, 7.5)% (p<0.001), TAR level 2 (time 0 vs time 3, 22 (7, 30) vs 5(3, 10)% (p<0.001), TBR level 1 (1 (0, 4) vs 0 (0, 0) p<0.001)) and TBR level 2 (0 (0.0, 1.0) vs 0 (0, 0) (p<0.001) was observed in patients treated with artificial pancreas. Conclusions: Subjects over 55 years with type 1 diabetic in treatment with an artificial pancreas system showed a significant improvement in the main metrics of glucose control after six months of follow-up.
